New Horizons Physical Therapy is an outpatient physical therapy clinic in New Albany and Jeffersonville, IN that specializes in orthopedics, sports medicine and worker’s compensation.

Our philosophy is to provide the most effective and advanced personal care for every patient. We feel that our one-on-one treatment approach is essential to providing the highest quality of care and secures the best outcome for our patients. We strongly believe that continuity-of-care is not only important to the consumer of healthcare, but should be a priority to the healthcare provider as well. We pledge to make every effort to ensure that you see the same physical therapist every visit.

At New Horizons our goal is for every patient to see the same physical therapist from the initial evaluation until discharge. We strive to treat every patient with our Golden Rule Service—as if they were a family member. We realize as a consumer you have a choice where you receive your rehabilitation care. We would be grateful and honored for the opportunity to assist in your recovery.

We accept most insurance including Medicare and Workers’ Comp.

Our licensed physical therapists in New Albany and Jeffersonville, IN are highly trained and dedicated to your personal care and recovery. We are committed on providing creative and effective individualized treatment intervention and progression with a strong emphasis on patient education and comprehension.

Dale Woolbright, PT, MHS, Clinic Director – New Albany
Dale has over 35 years of extensive clinical experience with emphasis on orthopedic/sports physical therapy, and vestibular rehabilitation. He enjoys treating the New Albany community and seeing his patients achieve their rehabilitative goals.

Jessica Farrel, PT, DPT – Jeffersonville clinic director
Jessica graduated in 2015 from Harding University. Jessica became a physical therapist due to a passion to help others return to activities they enjoy, stay independent and at home, and to enjoy life with less pain. She specializes in dry needling, sports/orthopedic injuries, and vestibular rehabilitation. Jessica enjoys treating pediatric patients and patients with disabilities as well. Her favorite joint to treat is the shoulder.

 

Laura Raulerson, PT
Laura graduated in 1992 from Maryville University in St. Louis, MO. She wanted to become a physical therapist after having therapy following a right ACL reconstruction in high school. She loves helping others improve function, decrease pain and stay active. She enjoys getting to work hands-on with patients to help them get back to enjoying everyday life. Her favorite body parts to treat are the knee and the neck

Yes. The choice of where you receive therapy is always yours. While the majority of our patients come to us by physician referral/recommendation, many clients request to see us due to our strong clinical reputation in the community.

We operate in a direct access state where, if your insurance plan authorizes self-referral to physical therapy, a physician referral is not necessary for you to be seen by a licensed therapist. Please check with your insurance though as many health insurance plans require a physician’s referral for therapy to be covered. We also offer treatments, services and/or products that do NOT require a referral and are available on a self-pay basis.

We participate in the majority of major health insurance plans including Medicare and Worker’s Compensation. Please contact us prior to your first visit to see if we participate with your specific health plan.

As a service to our patients, we will gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.

Your therapist will conduct an evaluation of your condition lasting between 60-90 minutes. The evaluation is necessary to create a program designed specifically to address your needs for a maximum recovery.

– Referral slip from your medical doctor
– Your medical insurance card
– Your ID
– If you are getting therapy as a result of a worker’s compensation claim or an automobile accident, you will also need to bring your claim information, including claim number.

Please arrive 15 minutes early on your first visit to fill out any necessary paperwork.

We recommend comfortable clothing such as sweatpants or shorts, a T-shirt or tank-top and sneakers.

Each visit will last 60 – 90 minutes on average. Times vary due to the treatment and exercises being performed.

The regularity of your visits will be determined after your initial evaluation, but most patients have 3 visits each week scheduled.
